NYS Tax Department no longer mailing annual sales tax forms
Since March 22, 2010, the New York State Tax Department http://www.nystax.gov/ no longer automatically mails Form ST-101 (New York State and Local Annual Sales and Use Tax Return).
In order to file and pay tax due returns, you can access the NYS Online Tax Center. This online
service is quick, free, secure, saves paper, and you’ll receive an online confirmation of your filing.
You can still file a paper Form ST-101 by downloading the form from the New York State Tax Department web site http://www.nystax.gov/ and mailing it in.
